Organization disability parking certificate

Organizations can apply for a disability parking certificate if they frequently transport clients with disabilities. This is most common for public or private social service agencies, care centers and nursing homes.

Certificates cannot be issued to taxi or limousine services. People who use those services should have their own disability parking certificate.

How to apply

There is a fee of $5 for each organization disability parking certificate requested. 

Organizations must submit a written statement on letterhead to Driver and Vehicle Services (DVS). The statement must:

• Include the organization's federal employer identification number (FEIN).
• Include the vehicle identification numbers (VINs) the certificates will be used in, when practical.
• State the certificates will be used solely for the benefit of a person who has a physical disability, per Minnesota state law.
• State the organization will report lost or stolen certificates to us within 24 hours of first discovery.
• State the organization will immediately notify us if the certificate is misused or suspected of misuse.
• Include a description of the internal controls to be used to ensure the proper use of certificates and to account for all issued certificates. 
• State the organization is aware that failure to abide by the above listed requirements may result in the revocation of all certificates.

Mail the written statement and payment to Driver and Vehicle Services, 445 Minnesota St., Suite 164, St. Paul, MN 55101-5164.

Organization parking certificates are valid for three years.

To renew, submit a new statement on letterhead. Make sure to include the 6-digit numbers of the current certificates and the organization’s FEIN.
